How Do I... Sick Notes

Get a sick note

SC2-formFor the first 7 days of an illness you should fill in a self-certification form - an SC2 - available from Social Services or from the reception desk. If you need to have a private certificate for this period you or your employer will have to bear the cost of this (currently £14.50).

A sick note can be issued without your having to be seen by a doctor if another doctor has seen you - such as at the hospital or from Devon Doctors Out of Hours Co-operative - and we have a letter from that doctor.

If the sick note is a continuation you may request one as below unless you have been told that you need to be reviewed in surgery.

  1. The day before your sick note runs out, contact the surgery (our 'phones are less busy after the morning surgeries).
  2. We will need the following information:
    • Your full name and address
    • The date you want your sick note to start from
    • Your medical condition, which is on your current sick note
    • The duration for which you need it eg: one week
  3. If you need a signing off note, you can request this by ringing or calling in, giving your name and address and the date you wish to return to work.
  4. Your sick note will be ready within 48 hours.
