If you wish to join our Practice please ask a receptionist for a registration form for each person that wishes to register. If you have your medical card available please attach it to the form. 

You may fill in the form straight away or take it home and return with it at a more convenient time. 

The form will ask you for personal details such as your name, address, date of birth, telephone number, previous address and doctor. This information helps the Health Authority to quickly trace your medical records and send them to your new doctor.

We will ask you to make an appointment for a "New Patient Check" and we will enquire about past and current illnesses, medication and lifestyle. We will also measure your blood pressure, height and weight as well as perform a simple test on your urine - please bring a sample. Your registration details will then be entered onto our computerised system and when you need an appointment in the future we will be able to action your request quickly and accurately.
