If you require transport to the Hospital in order to attend for an out patient appointment or a planned admission, then the Reception staff will be pleased to help you.

If you wish to travel using the Hospital Car Service you will need to complete a small form, which can be obtained at the surgery. Once you have completed the form you can either hand it back to the Receptionist or post it directly to the Hospital. You should ensure that the form is completed at least 7 days before the transport is required.

If, for medical reasons, you need to travel to your appointment/admission in an ambulance, please contact the Surgery either by telephone or in person in order to make the necessary arrangements. Remember that the surgery staff will need as much information as possible about your appointment/admission in order to make the booking.
